Submit mandatory immunization records

All entering students must send copies of official immunization records showing they have received the required 2 dose series of Measles, Mumps, and Rubella (MMR) vaccine to the SSU Office of Admissions and Records prior to enrolling at SSU. Those who will be under age 19 on January 22, 2018 must also send records showing they have received the required series of 3 doses of Hepatitis B vaccine. Compliance is a mandatory condition of enrollment and should be completed no later than December 9, 2017. Students unable to locate immunization records may send copies of blood test results confirming immunity to the diseases prevented by the required vaccines.

Read more now about the immunization requirements and how to satisfy them. Attach these records to the completed Immunization Form and send promptly to: SSU Immunization Requirements, Fall 2017; Office of Admissions and Records; 1801 E. Cotati Avenue Rohnert Park, CA 94928.

Students who do not comply with this pre-enrollment requirement on time will have a Hold on their ability to register for classes and are subject to other late compliance penalties. The CSU is considering adding additional immunizations to this requirement. See the above immunization requirements link for details - in the frequently asked questions section.

Note: The CSU is in the process of revising immunization requirements that will likely add additional immunizations to the current list, in accordance with the recommendations from the U.S. Center for Disease Control (CDC).
